You may wish for extra coverage for more peace of mind. Eligible employees may purchase additional Voluntary Life and AD&amp;D insurance. Premiums are paid through payroll deductions.<\/p>\n&#8211;\u00a0<strong>You:<\/strong>\u00a0Increments of $10,000<br \/>\n&#8211;\u00a0<strong>Spouse:<\/strong>\u00a0The lesser of 50% of the employee&#8217;s Voluntary Life amount or $250,000<br \/>\n&#8211;\u00a0<strong>Child:<\/strong>\u00a0Birth to 6 months: $1,000; 6 months to age 25: $10,000<br \/>\nCertain levels of life coverage may require you to demonstrate good health by completing an Evidence of Insurability (EOI) form. In these cases, your coverage will not take effect until approved.\n<p><strong>Age reduction provision:<\/strong> If you have reached age 65, you and your spouse&#8217;s amount of life insurance will be 65% of the amount of life insurance you had prior to 65.
