Coverage Overview
Our health coverage runs from Jan. 1 - Dec. 1 of each year. Employees, spouses, and children who meet the eligibility requirements below are eligible for coverage by our benefits program.
For new hires or newly benefits-eligible employees, you have 30 days from your date of hire or benefit eligibility date to complete your enrollment.
Once completed, your coverage begins the first of the month following 60 days of employment. If you do not complete your enrollment within this timeframe, you will not be able to elect benefits until the next scheduled annual enrollment period or within 30 days of a qualifying life event.
Dependents
Our health coverage runs from Jan. 1 - Dec. 1 of each year. Employees, spouses, and children who meet the eligibility requirements below are eligible for coverage by our benefits program.
Dependents eligible for coverage include:
- — Your legal spouse (or common-law spouse where recognized)
- —Children up to age 26 (includes birth children, stepchildren, legally adopted children, children placed for adoption, foster children, and children for whom you or your spouse have legal guardianship).
- — Dependent children 26 or more years old, married or unmarried, and primarily supported by you and incapable of self-sustaining employment by reason of mental or physical disability which arose while the child was covered as a dependent under this plan (periodic certification may be required).
Verification of dependent eligibility may be required upon enrollment.
